Community Integrated Care has delivered innovative services to people with learning disabilities since 1988. We are one of Britain’s biggest and most successful social care charities, providing specialist support across England and Scotland.
Our services for people with learning disabilities include:
- Supported living
- Registered Care Services
- Nursing care
- Floating support
- Supported employment
- Respite and short breaks
Our learning disability teams support people with a host of needs to lead full and independent lives. From providing small packages of community-based support to people who are very independent, to promoting the health and happiness of people with more complex learning and physical disabilities, we can provide support that is right for you.
We are committed to delivering genuinely person-centred services. We focus on really getting to know people – how they want to be supported, what they want to achieve in life and what is important to them – to offer support that is as individual as they are.
Every person we support has an outcome-focused support plan, which looks at how we can work together to make their aspirations a reality. This might include goals around being healthy, accessing the community, staying connected with family and friends, or learning new skills.
Our teams work closely with a wide range of professions – from Community Learning Disability Teams, to speech and language therapists and occupational therapists. We also have great experience in working with housing providers to develop exceptional supported living services and care homes.
Led by the People we Support.
Our charity is committed to social inclusion - we think that it is vital to listen and respond to the views of the people we support. We regularly host Person-Centred Reviews with the people we support. These see us work with the person, and the important people in their lives, to look at what is working for them and what we can make better.
We also have a fantastic team of Peer Reviewers in every region we provide support. These are people we support, who are trained and employed as ‘experts by experience’. They review our services from the perspectives of people who receive them, to help ensure that we’re delivering the personalised care that we aspire to.
Community Integrated Care also has Voice Groups. These are regional and national forums, which see people we support share their views and help us set our organisational plans.
A Proud History
Our charity has been delivering specialist learning disabilities for almost 30 years. We were one of the pioneers of the Care In the Community Agenda and supported the first person with a complex learning disability to live independently in the community.
Community Integrated Care has been recognised at the likes of the National Care Awards, National Learning Disability Awards and 3rd Sector Care Awards.
