Personalised Care and Support
We are committed to delivering genuinely personalised services. We focus on really getting to know people – how they want to be supported, what they want to achieve in life and what is important to them – to offer support that is as individual as they are.
We also regularly host Person-Centred Reviews with the people we support. These see us work with the person, and the important people in their lives, to look at what is working for them and what we can make better.Our colleagues are trained to adopt a number of leading person-centred planning tools, which have been developed by our partners at Helen Sanderson Associates. These help us to understand what the people we support want from life and how we can best enable them to achieve their goals. In every service both our colleagues and people we support actively use One-Page Profiles, to ensure that we’re focusing on people’s goals and creating great teams.
The Voice of People We Support
Our charity is committed to social inclusion. We also have a fantastic team of Peer Reviewers in every region we provide support. These are people we support, who are trained and employed as ‘experts by experience’. They review our services from the perspectives of people who receive them, to help ensure that we’re delivering the personalised care that we aspire to.
Community Integrated Care also has Voice Groups. These are regional and national forums, which see people we support share their views and help us set our organisational plans.
Working in Partnership
We are committed to working in partnership with health and social care commissioning partners to develop integrated services that make the biggest impact possible on the communities we work in. Our charity also has thriving relations with housing providers across England, enabling us to create new services and ensure that the people we support enjoy living in high-quality accommodation.
Nationally, we are proud to be active members of Association for Real Change, Voluntary Organisations Disability Group and SITRA.
