be supported, what they want to achieve in life and what is important to them.
We provide consistent, long-term, reliable complex care and constantly strive to deliver the highest quality support. We recognise the complexities of supporting people within their own home and understand the sensitivity required to ensure that a stable and comfortable environment is always maintained.
As well as offering tailored support solutions that are as unique as each person we support, we offer a holistic full-service approach to complex care conditions, including:-

- Specialist training and ongoing support for teams
- Practical support for our Commissioners and Case Managers
- Comprehensive reporting pre-litigation
We recognise how sensitive and specialist complex care support and rehabilitation is and we ensure that our support is delivered through highly skilled teams who have the right experience and knowledge. Each team member undertakes bespoke training programmes centred on the individual needs of each person supported and we have our own dedicated, in-house Learning & Development function, to support this commitment.

Our approach is a co-produced pathway connecting people back to their lives, loved ones and communities promoting independence through bespoke provision into work, education and activities.
Jack is an independent 24-year-old who was involved in a roadtraffic accident. The accident left him with a spinal cord injury. But Jack didn’t let this life-changing injury hold him back. Following an intensive rehabilitation programme, Jack become passionate about regaining his independence, developing new skills and living the best life possible. With support from Community Integrated Care’s specialist team, Jack began attending college one day a week, completing a welding course.
In 2017 Jack proudly left his family home to live in his own accommodation, increasing his support to six days a week. The flexibility and guidance from Community Integrated Care enabled Jack to become more confident and self-sufficient in his everyday life - cooking, cleaning and socialising independently. By January 2019 Jack’s confidence had grown massively, and with the support of his Care Team and Case Manager, he gained the independence he always wanted and we’re proud to say he no longer requires support from Community Integrated Care.
